Former patients wishing to have medical records sent to another physician and/or another psychiatrist may request a Release of Information Form via our contact page. You may ask to receive a copy of your medical record (paper or electronic) as needed. Health records from this practice are maintained in accordance with state and federal law by Britt Gonsoulin, MD, MPH. Records will be destroyed no less than 10 years after the last date of services received from this practice, although most records will be held for more than 10 years from the last date of service.

Please note:

In accordance with applicable state law, you may be charged for the copying and transmittal of the records. 

For minors, records shall be held for 3 years beyond the date they turn 18 or 10 years from the last date of service (whichever is longer).
